CTE, or chronic traumatic encephalopathy, is a progressive brain disease caused by repeated head trauma. It can only be confirmed after death, through an autopsy that finds a specific pattern of abnormal tau protein in brain tissue.
What is CTE?
Chronic traumatic encephalopathy is a neurodegenerative disease tied to years of head impacts, not a single blow. Neurologist Harrison Martland first documented it in the 1920s, describing “punch drunk” boxers who developed slurred speech and worsening memory after long careers. The condition was later renamed dementia pugilistica, then chronic traumatic encephalopathy in 1949.
In MMA, the same exposure builds up in the gym as much as in the cage, even when no single hit produces a knockout. Inside the brain, it’s linked to a buildup of an abnormal protein called tau, which clumps around blood vessels deep in the brain’s grooves and gradually kills nerve cells. That buildup, not any one concussion, is what defines CTE.
Most fans confuse CTE with a concussion. A concussion is a single injury that usually resolves. CTE is a disease that develops after years of accumulated trauma, including hits that never produced concussion symptoms at all.
How CTE develops in MMA
Every strike that reaches the head, whether it knocks someone down or barely registers, jostles the brain inside the skull. Punches, elbows, knees, and ground strikes all count, and so do the impacts absorbed in the gym long before fight night. Researchers call hits that don’t produce dizziness or confusion “subconcussive,” and evidence suggests they still contribute to long-term damage even though nobody notices them at the time.
Not every combat sport loads the brain the same way. A 2020 Cleveland Clinic study tracking active boxers and MMA fighters found boxers lost brain volume at a faster average rate, 145 cubic millimeters a year, than MMA fighters, who lost about 100 cubic millimeters a year. Fighters with no history of head trauma gained volume over the same period. Researchers said the pattern needs more study, but it points to fight discipline and total head-impact exposure as factors in long-term risk.
CTE vs. concussion
Most people land on this term right after hearing “concussion” and want to know if the two mean the same thing. They don’t, and the difference matters for how each one gets diagnosed and treated.

A fighter can survive several diagnosed concussions and never develop CTE, just as years of undiagnosed subconcussive exposure can still lead to it. Researchers use a related term, traumatic encephalopathy syndrome (TES), for the clinical picture a doctor can observe in a living fighter, since CTE itself can’t be confirmed until after death.
Symptoms and stages of CTE
CTE symptoms tend to follow a rough pattern. Mood and behavior changes, growing irritability or a shift toward depression, often show up first, sometimes while a fighter is still competing. Cognitive problems, memory lapses or trouble planning ahead, tend to surface later, usually years after retirement. Movement is affected in more advanced cases, with tremors and slower, less coordinated speech becoming common, and dementia can follow in the most severe stages.
Pathologists grade the physical damage on a four-stage scale, from stage I (mild) to stage IV (severe), based on how much tau has built up and where it has spread. That staging has no bearing on how a fighter seems day to day. It’s only assigned after an autopsy, well past the point where it could change anything about their care.
How CTE is diagnosed
There’s no scan, blood test, or exam that can confirm CTE in a living person. The only way to diagnose it is by examining brain tissue under a microscope after death, checking for a specific pattern of tau buildup around blood vessels deep in the brain’s folds.
While a fighter is alive, neurologists look for traumatic encephalopathy syndrome instead: a clinical diagnosis built around a documented history of repeated head impacts, plus cognitive, mood, or behavioral changes lasting at least a year that can’t be explained by another condition. Consensus criteria published by a National Institute of Neurological Disorders and Stroke-backed panel in Neurology in 2021 require at least six years of exposure to contact sports or repeated head trauma before TES can even be considered. Meeting the criteria doesn’t guarantee a fighter has CTE, but it’s the closest marker researchers have while someone is still alive.
How common is CTE in MMA?
Large-scale data on MMA-specific CTE rates doesn’t exist, since confirming the disease requires an autopsy and most fighters are still alive. Researchers instead track the closest living proxy: traumatic encephalopathy syndrome.
A 2022 study in the British Journal of Sports Medicine, drawing on Cleveland Clinic’s Professional Athletes Brain Health Study, found that 41% of 176 boxers and MMA fighters met the criteria for TES, with age and career fight count as the strongest risk factors. A separate 2023 analysis of 130 fighters from the same research program, published in Neurology, found a similar rate of 40%, and those fighters showed a faster pace of brain volume decline on repeat MRI scans.
The same research group has also found signs that some damage isn’t fixed in place. In a 2022 study, 45 fighters who’d been out of competition for at least two years were tracked against 45 still-active fighters over three years, using annual MRI scans and cognitive testing. Memory and motor-speed scores climbed in the retired group, alongside gains in executive function. The active group mostly held flat, with a slight dip.
